Geriatric Care Management
- Placement of in-home caregivers, providers, & companions
- Advocate for the elder in the medical & social systems
- Management of caregivers payroll
- Coordinate periods of rehab following a hospital discharge
- Arrangement of home repair and housekeeping assistance
- Assisting with a move to a new place of residence -- home, apartment, skilled nursing facility, etc

- Refer hospitals or nursing home providers
- Search for and recommend senior housing communities
- Referral of banks or trust officers for estate planning issues
